Issue of a Replacement for Lost Death Certificate

This service allows the customer to submit a request to obtain a death certificate when the original death certificate is lost.

Service Process


Conditions & Requirements

A relative of the deceased must be present to obtain the certificate.

Copy of the death certificate, if any.

ID card of the certificate recipient.

Passport, ID or Family book (for Nationals).

In the case of companies, a representative of the company must be present.

Request must be made at the same preventive medical center from where the original death certificate was issued.
