This service allows customers to request the issue of a replacement for an original birth certificate.
Submit the application through the e-system and pay the fees or submit the application through public health centers.
Once the documents and original copies are verified, the birth certificate will be issued.
The certificate should be collected by the father or his representative from public health centers or the customer can request an electronic copy via email.
The presence of the father and the mother or an authorized delegated person is required.
Submit all original documents including passport of the mother or father and the person concerned.
If the applicant for a replacement certificate is currently living abroad, copies of original documents are required in addition to the power of attorney of a representative assigned to receive the certificate. If they don’t have any relatives to receive the replacement certificate for the birth certificate, the staff of the concerned consulate can receive it with a letter indicating his authorization for that person to receive the certificate on his behalf, stamped by the embassy.
You must contact the center from which the original birth certificate was issued to receive a replacement for the lost one.
The application can be submitted through EHS website or public health centers with all the required documents.
